The Georgia Board of Dentistry does not license dental service organizations (DSO), clinics, offices, or any other non-person entity.
You must provide the full name or license number of the individual dentist/dental hygienist against whom you are filing this complaint.

The authority of the Georgia Board of Dentistry is limited to sanctions against the license of a dentist or dental hygienist. The Board does
not have authority to settle fee disputes, nor does it settle personality conflicts. Please search for the licensee you are filing your
